Provider Partners Connect Care ACO REACH program was founded by a physician for physicians to deliver high-quality, coordinated care for Medicare Fee-for-Service Beneficiaries.
With a focus on high-needs patients, we help clinicians improve outcomes and simplify value-based care. Primary Care Physicians benefit from a payment program that includes a premium above the Medicare Fee Schedule for E&M codes, along with annual incentive payments based on shared savings tied to improved quality and reduced hospitalizations.
