Provider Partners Health Plan partners with Skilled Nursing Facilities and Assisted Living Facilities to offer tech-enabled value-based care. Provider Partners’ clinical model brings a Nurse Practitioner and RN Care Coordinator onsite several times per week.
Skilled Nursing Facilities benefit from a 100% share of savings based on lower expenses from improved outcomes and fewer unnecessary hospitalizations.
Residents enjoy supplemental benefits not offered by traditional Fee for Service Medicare including dental care, podiatry, hearing and much more.
ACO REACH for Providers
Provider Partners Connect Care ACO REACH was founded by a physician for physicians, and is focused on delivering high-quality coordinated care for Medicare FFS Beneficiaries.
Both high needs eligibility and standard eligibility programs are offered.
Primary Care Physicians are offered a payment program that includes a premium on top of Medicare Fee Schedule for E&M codes plus an annual incentive payment from shared savings based on improved quality and reduced unnecessary hospitalization.
